Please read the following document to ensure the proper procedure for obtaining a Special Diet Order.

This form is the Iredell Statesville Schools Special Diet Order Form.  The purpose of this form is to record the student's disability requiring dietary modifications of school meals and the changes needed.  Examples of modifications possibly needed are:  Texture modifications, Nutrient modifications, Diabetes, or Food Allergies.  The form must be filled out completely and signed by the parent/guardian and Licensed Physician.  Please follow the above procedures to ensure the diet order is received.
