Lakeshore Middle School's SAP Coordinator's mission statement begins with assisting student's towards having a successful school career by achieving emotional, social, and academic health.
About Me
Mrs. Forquer earned her Master's Degree in Clinical Social Work from the University of Tennessee and is a Licensed Clinical Social Worker, with 15 years of experience in the mental health field. She has served as children and youth clinical coordinator for a community mental health agency, an outpatient psychotherapist, worked in the substance abuse community, and has worked with "at risk" and pregnant youth.
SAP (Student Assistance Program) is a program designed to assist students with their social, emotional, and academic development. The primary focus of SAP is early identification and intervention of problems which may impede a students successful journey through school. Common issues students may face include ADHD, depression, substance use/abuse, legal problems, medical problems, home problems, and social concerns.
Identification of issues at an early stage can involve the following:
clinical assessment via checklists
one on one communication & supportive counseling with student
parental interviews
referrals from teachers
threat assessments
substance abuse assessments
Intervention may include the following:
support groups
peer mediation
one on one counseling
family counseling
referral to community support systems and agencies
threat assessments
substance abuse assessments
SAP CORE TEAM consists of individuals specially trained to facilitate support groups. Core team members refer students to the SAP program, serve as a case manager/mentor for students, facilitate groups, and gather information which helps in assisting the individual student. Case Managers on the core team are vital to the SAP program. A case manager/mentor is seen as a support person for the student referred to SAP. The student can discuss any problems of concern regarding home and/or school and feel that this information is kept confidential.
SUPPORT GROUPS are led by trained SAP core team members, volunteers from outside support agencies, and the SAP Coordinator. A number of groups are offered each semester and may include:

PEER MEDIATION PROGRAM is a program designed to allow students to peacefully resolve conflicts with other students. Students are nominated and selected by their grade level teachers to become a Peer Mediator. The students nominated must possess qualities such as dependability, honest, trustworthy, age appropriate, and empathy, just to name a few.
CONFIDENTIALITY is the cornerstone of the SAP program what sets SAP apart from the rest of the school. It is maintained in support groups by students, the SAP Coordinator, the SAP Core Team and any who are directly involved with the SAP program.
REFERRALS to the SAP program can be made by a telephone call to Mrs. Forquer or as written referral. Students, teachers, parents, friends or administration can refer to the program. If you feel someone needs the support of SAP, please do not hesitate to call.
